When you are in the middle of a chronic illness, two things are almost always missing: the money to try something different, and the community to remind you that you are not alone. Hoff's Heroes exists for both.
How it works
We accept nominations on a rolling basis. Our team sits down with each nominee — in person when we can, on video when we can't — and learns their story: what they're facing, what they've tried, and what they want to try next.
Then two things happen. First, we share their story. Working with a professional production team, we produce a short film in partnership with the hero and their family — a piece of their journey, told the way they want to tell it. Second, we award them a wellness grant of $10,000 to $20,000 toward a specific health goal they define. That might be an integrative clinic, a retreat, a home-gym setup, a nutrition program, a specialist outside their insurance network, a caregiver, an adaptive device — their choice, their plan.
What we mean by "hero"
Not someone who is winning. Someone who is trying. The people we've met along our own journey — the ones who keep choosing to believe something can still change — have been the single biggest source of hope for us. Hoff's Heroes is how we pass that hope forward.

Who can be nominated
- Anyone living with a chronic illness — progressive, autoimmune, rare, or undiagnosed.
- Individuals, caregivers, or families supporting a loved one.
- Any age. Any state. Any background.
- Self-nominations are welcome and encouraged.
What grants cover
- Integrative and functional-medicine clinic intensives
- Wellness retreats, detox programs, and healing centers
- Nutrition coaching, specialty foods, and home kitchen setups
- Breathwork, sauna, cold therapy, or hyperbaric equipment at home
- Specialists outside of conventional insurance coverage
- Caregiver relief, adaptive equipment, accessibility modifications
